You can either use the menu or console/chat commands to warn a player.

Access the menu with:

Console Command: gex_warnings
Chat Command: !warnings

Warn a player with:

Menu
Console Command: gex_warn <steamid64> <reason>
Chat Command: !warn <nick> <reason>
